Important notes

• If you wish to reinstate your monthly deductions using your existing GIRO arrangement, please use the Reinstate GIRO deduction for MediSave contributions form.
• Applications submitted on the 15^th of the month or earlier will take effect on the 25^th of the same month. Otherwise, it will take effect from the following month.
• If you want to use a bank account that doesn't belong to you, the bank account holder must read and accept the declaration in this form.
• If you're an undischarged bankrupt, you must first obtain approval from the Official Assignee before you submit this form.
• Before you begin, ensure that you have updated your contact details in Account Settings.

To complete this form, you will need:

• A personal bank account with one of the eGIRO participating banks.
• Your bank account number.
• Your internet banking credentials.
