Application to join CareShield Life

Use this form to apply for CareShield Life, a long-term care insurance scheme that provides basic financial support should an insured develop severe disability.

If you wish to help your family member to apply for CareShield Life, you will need them to login to their Singpass. The application should be made by the insured, as liability of premium payment and enforcement falls on the insured. Due to data privacy concerns, an insured's premium information should not be disclosed to a third party.

Important notes

  1. This form may take you 15 minutes to complete.
  2. You should check the estimated CareShield Life premiums on the CareShield Life Premium Checker before starting the application.
  3. Please complete the disability declaration truthfully. It is an offence under the CareShield Life and Long-Term Care Act 2019 to knowingly make or provide a false declaration. An offender on conviction of an offence under this Act may be liable to a fine not exceeding $5,000 or to imprisonment for a term not exceeding 12 months or to both; and in addition, to a financial penalty.
  4. Please note that your CareShield Life Policy will be automatically terminated if you renounce your Singapore Citizenship or Singapore Permanent Residency.
  5. Once your CareShield Life cover commences, you will not be allowed to terminate your CareShield Life cover after the free-look period, which is 60 days from the cover commencement date.
