The medical institution that you approach for completion of the Medical Assessment Report must submit the completed report to CPF Board within 90 days of your application. Please note that completed report submitted through other means, including by you, may not be accepted and will delay the processing of your application. Do allow six to eight weeks for medical institutions to submit the form upon your request.
Please note that your application will be cancelled if we do not receive the completed report from the medical institution by the given deadline.
