How do I submit the "MediShield Life and CareShield Life Health Declaration for Permanent Residence Applicants"?
All new Singapore Permanent Residence applicants who have received In-Principle Approval are required to declare their health condition for MediShield Life and CareShield Life before completing formalities with the Immigration & Checkpoints Authority (ICA). The health status declared will not affect their Singapore Permanent Residence application.
 
The health declaration can be submitted via the 'MediShield Life and CareShield Life Health Declaration for Permanent Residence Applicants' form on the CPF website. Upon receiving In-Principle Approval from ICA, please wait one day before submitting the form. Each Permanent Residence applicant must submit a separate individual form. Singpass login is required.
 
Applicants aged 21 or older
If you are the Permanent Residence applicant submitting the health declaration yourself, please log in using your Singpass and select ‘Myself’ under the ‘Select Person to be Insured’ section. If you do not have a Singpass account, please apply for one. If you are unable to apply for a Singpass account, please have a family member who is a Singapore Citizen or Permanent Resident log in using their Singpass and select ‘A Family Member’ under the ‘Select Person to be Insured’ section. 
 
Applicants under aged 21 
Applicants under aged 21 must have a parent or legal guardian submit the health declaration on their behalf. If the parent or legal guardian is a Singapore Citizen or Permanent Resident, they must log in using their Singpass and select ‘A Family Member’ under the ‘Select Person to be Insured’ section.
 
No Singpass
If you are unable to apply for Singpass or do not have a family member who is a Singapore Citizen or Permanent Resident with Singpass who can submit the health declaration on your behalf, please use this form instead. This form is intended for applicants who do not have Singpass. Unsolicited submissions from applicants with Singpass will not be processed. 
 
After Submission
You can check the status of your health declaration via "Activities". If you are required to attend a disability assessment, the Agency for Integrated Care (AIC) will notify you of the details, or you can contact AIC at apply@aic.sg. Please attend the assessment promptly within the specified timeframe. You will be informed of the outcome via email, SMS or hardcopy once your application has been processed.
 
Please ensure that you have submitted all required documents and completed the administrative requirements specified in ICA’s In-Principle Approval notification before your Completion of Formalities appointment at ICA.
