The fees for clinic-based and non-clinic-based severe disability assessments are $100 and $250 respectively.
If you are applying for CareShield Life:
The fee for your first severe disability assessment will be waived, regardless of the outcome. For any subsequent assessments, you will only be reimbursed if you are assessed to have a severe disability.
If you are applying for other severe disability schemes (i.e., ElderShield or MediSave Care):
You will need to pay the assessment fee upfront to the MOH-accredited assessor. The fee will be reimbursed if you are assessed to have a severe disability.
