When do I need to submit the documents (medical certification and employment information) to support my application for Dependants’ Protection Scheme claim?

Both your doctor and employer are required to submit the completed medical certification form and your employment information respectively to CPF Board within 90 days of your application. The medical certification form and employment information will be used to process claims under Dependants’ Protection Scheme (DPS) and Home Protection Scheme (HPS), if you have applied for both.

Please note that your application will be cancelled if we do not receive the supporting documents by the given deadline.

If you are applying for DPS benefits only, you can submit the supporting documents to the Great Eastern Life directly.
