MediSave contributions made via this form are not considered voluntary top-ups, and will not qualify for dollar-to-dollar matching under the Matched MediSave Scheme.

Use this form to request an instalment plan or make contributions on your outstanding MediSave payable.

If you do not have any outstanding MediSave payable, you can make contributions to offset your future MediSave payable as a self-employed person.

Important notes:

  • If you are an undischarged bankrupt, you must obtain approval from the Official Assignee before using this form.
  • The amount of MediSave contributions made for the Self-Employed Scheme may differ from what is required to qualify for the Workfare Income Supplement Scheme.
  • MediSave contributions made via this form are not considered voluntary top-ups, and will not qualify for dollar-to-dollar matching under the Matched MediSave Scheme.
  • This service is not available on the last day of every month, 10PM, to the following day, 12:30AM.
  • To avoid delays in crediting of your contributions, please do not submit PayNow QR payments via banking applications during this period.

To complete the form, you will need:

  • Your banking app, payment app, or authentication token, if you are using PayNow.
  • Documents showing your income and expenses, if requesting an instalment plan.
