T&C for health declaration by permanent resident

Please read the following important terms and conditions carefully before you submit this declaration:

 

  1. Please complete the health declaration truthfully. It is an offence under the MediShield Life Scheme Act 2015 (Act 4 of 2015) and CareShield Life and Long-Term Care Act 2019 (Act 26 of 2019) to knowingly make or provide a false health declaration. An offender on conviction of an offence under either Act may be liable to a fine not exceeding $5,000 or to imprisonment for a term not exceeding 12 months or to both; and separately, to a financial penalty.

  2. This health declaration, once processed, will supersede any previous MediShield Life and CareShield Life Health Declaration(s) relating to the Person To Be Insured for the assessment under MediShield Life. However, any previous declarations may continue to be taken into account, together with this health declaration, for the assessment under CareShield Life.

  3. The Person To Be Insured’s health information will be shared with other public authorities, or entities or persons involved in the administration of the MediShield Life Scheme and/or CareShield Life Scheme (known collectively as “Recipients”), to facilitate the underwriting for the MediShield Life and/or CareShield Life Scheme.

  4. The Person To Be Insured may choose not to declare his/her health status or share his/her health information provided in this health declaration form with the Recipients for the purpose of underwriting for the MediShield Life Scheme and/or CareShield Life Scheme by writing to us for the relevant form.

    For MediShield Life - The Person To Be Insured will have to pay 30% Additional MediShield Life Premiums for 10 years as it cannot be ascertained that he/she does not have any serious pre-existing health conditions.
    For CareShield Life - In addition, the Person To Be Insured (if born in 1979 or earlier and obtained permanent residency on or after CareShield Life is launched on 1 October 2020), may be denied claim payment from CareShield Life if he/she is found to be severely disabled and unable to perform 3 or more ADLs before his/her permanent residency grant date. His/her CareShield Life coverage will be cancelled and all CareShield Life premiums paid will be refunded without interest.

  5. The Person To Be Insured’s health information held in Government databases, databases of entities involved in the administration of MediShield Life and/or CareShield Life, and/or databases of any healthcare institution, if any, will form part of the assessment under MediShield Life and CareShield Life by any entity involved in such assessment.

  6. The Board may, by notice, change these terms and conditions from time to time by either posting the new terms and conditions on cpf.gov.sg. Unless indicated otherwise, the changes shall apply and will be effective as of the date the changes are posted on cpf.gov.sg. You are advised to check cpf.gov.sg regularly. We may also choose to inform you by other means of communication. Your continued participation in MediShield Life and/or CareShield Life or the use of the Board’s services constitutes acceptance of the new terms and conditions.
