Make MediSave top-up to employees and platform workers

Top up your employee's or platform worker's MediSave for future healthcare needs.

Use this form to submit your request to make voluntary cash top-ups to your employee’s or platform worker’s MediSave Account. Top-ups made through this form are tax-deductible for the employer and platform operator. This service allows you to make cash top-ups up to your recipient’s Basic Healthcare Sum.

Please note that this form is not for making contributions under the Additional MediSave Contribution Scheme (AMCS). If your intent is to make contributions under AMCS, please submit your request via CPF EZPay using your existing CSN with the payment code “AMS”.

It is an offence to make any false statement for any purpose connected with the Central Provident Fund Act.

Important notes

  • Top-ups are irreversible and cannot be returned to you. 
  • After submitting this form, CPF Board will contact you with further instructions to complete the top-up to your recipient’s MediSave Account. 

To complete the form, you will need: 

  • Your company's Corppass. If you do not have Corppass, you may write to us for an alternative application mode. 
  • Your Employer's CPF Submission No. (e.g., XXXXXXXXXX-PTE-XX), or Platform Operator's CPF Submission No. (e.g., XXXXXXXXXX-PWA-XX).
  • Your recipient's name and NRIC number.
