  • All fields marked with * are mandatory.
  • You will need to provide a valid identification number (For CPF members, NRIC/CPF Account Number and for non-CPF members, FIN/Foreign Passport Number) and a Singapore-registered mobile phone number to submit a callback appointment.
  • Callback appointment are subject to availability.
  • Please note that you can only have one callback appointment at any one time.
  • The callback appointment is created/changed/cancelled instantly upon completing the form.
  • Please change or cancel your callback appointment if you are unavailable at your chosen time slot.
  • A callback appointment attempt will be made to your mobile number at your chosen time slot. If you have missed our call, you may either call us back later or submit another callback appointment.
