How do I submit the "MediShield Life and CareShield Life Health Declaration for Permanent Resident Applicant" form?

Please submit the MediShield Life and CareShield Life Health Declaration for Permanent Resident Applicant form online and log in using your Singpass.

Declaring for Self
If you are the Person To Be Insured and you are aged 21 years or older, please select the option to declare for “Myself” under the section - Selection of Self or Family Member Declaration.

Declaring for family member
If you are aged below 21 years, please get your parent/legal guardian to log in using his/her Singpass to the e-Service and select option to declare for “Family Member’ to declare the health status on your behalf.
If you are aged 21 years or older but do not have a Singpass, please get your family member to log in using his/her Singpass and select the option to declare for ‘Family Member’ to submit the HD form on your behalf.

No Singpass
Alternatively, if you are not able to apply for Singpass or do not have a family member with Singpass that can help you submit the HD form online, you can write to us.

Form processing Time
After you have submitted your health declaration, please note the following:

  • Your health declaration will generally be processed within seven working days. You can check the status of your health declaration via “My activities”.
  • If the status is still showing ‘Pending review’ after seven working days, the Agency of Integrated Care (AIC) has evaluated your declaration and requires you to undergo a disability assessment. AIC will notify you of the details, or you can contact AIC at apply@aic.sg.
  • If you are required to attend a disability assessment, please do so as soon as possible within the timeframe specified in AIC’s letter. After the assessment, your assessors will submit their report to AIC. Only after AIC has evaluated the report will the status of your health declaration be updated to “Completed”, and you can then schedule an appointment with ICA.

Completion of Formalities with the Immigration and Checkpoints Authority (ICA)
Individual applicants should only schedule an appointment with ICA after their health declaration status shows “Completed”.
For family unit applications under the same ICA reference number, please ensure that all members have a “Completed” health declaration status before scheduling an appointment with ICA.
