The deceased member’s spouse, parents, child (if age 21 or older), or donee/deputy can sign the form at the hospital. You are encouraged to do this as soon as possible as hospitals have to submit the MediSave claim before the deceased member’s MediSave is distributed (typically within two weeks of the member’s passing). If the deceased member’s MediSave has been distributed, you will not be able to use his MediSave for the last hospital bill.
If there is no immediate family or donee/deputy, a caregiver who has been taking care of the deceased member during his lifetime may write to the Ministry of Health through the hospital to seek approval for the use of the deceased member’s MediSave for the last hospital bill.
