What are the documents that CPF Board requires if the Court has accepted an application to make or revoke a nomination and how should these documents be submitted?

Make a CPF nomination for members who lack mental capacity

If the Court accepts the application to make a CPF nomination on behalf of a member, the court may appoint a person to carry out the making of the nomination.

If you are the person appointed by the Court to carry out the making of the nomination, please complete the relevant CPF nomination form and indicate that you are the authorised person ordered by the Court to make the CPF nomination on behalf of the member who lacks capacity. The CPF nomination form must be affixed with the Court's Seal.

The witnesses to the nomination may be our Customer Service Executives (CSEs) at any of our Service Centres. If 2 other witnesses are chosen, please note that the witnesses to the CPF nomination must be at least 21 years old and not lack capacity. Please note that the witnesses cannot be the nominees or the authorised person signing on behalf of the member.

We encourage you to hand-deliver the completed CPF nomination form (affixed with the Court's Seal), with a certified true copy of the Order of Court and your NRIC to any CPF Service Centre. Alternatively, you may wish to send the documents to the Board by post. You can mail the documents to:

Revoke a CPF nomination for members who lack mental capacity

If the Court accepts your application to revoke the CPF nomination, and makes the order revoking the CPF nomination, please furnish the Board with a copy of the Order of Court and your NRIC (both documents to be certified as true copies by your lawyers).

We encourage you to hand-deliver the completed CPF revocation form (affixed with the Court's Seal), with a certified true copy of the Order of Court and your NRIC to any CPF Service Centre. Alternatively, you may wish to send the documents to the Board by post. You can mail the documents to:
