Form CPFIS/FMC/1

This form allows you to apply for the inclusion as a FMC or Insurer under CPFIS.

​Form CPFIS/FMC2

This form allows you to submit the qualifications and experience of qualifying fund manager during the application for inclusion as a FMC under CPFIS or whenever there is change in the qualifying fund managers. During application for inclusion as a FMC under CPFIS, there should be at least 3 qualifying fund managers.

Form CPFIS/FMC3

This form allows you to submit the amount of funds managed in Singapore and the name of the three qualifying fund managers so as to confirm continuing compliance with the admission criteria.

Form CPFIS/AF

This form allows you to apply for the inclusion of Authorised Fund under CPFIS.

Form CPFIS/RF

This form allows you to apply for the inclusion of Recognised Fund under CPFIS.

​Form CPFIS/ILP1

This form allows you to apply for the inclusion of Investment-Linked Insurance Products (ILPs) under CPFIS.

​Form CPFIS/IP1

This form allows you to apply for the inclusion of Non-linked Insurance Products (Endowment or Annuity) under CPFIS.

FORM CPFIS/SB

This form allows you to apply for the inclusion of Shares, Employee Share Option Scheme, Property Funds or Bonds under CPFIS.

​FORM CPFIS/FD

This form allows you to apply for the inclusion of Fixed Deposit Bank under CPFIS.

​FORM CPFIS/IA

This form allows you to apply for the inclusion as Investment Administrator under CPFIS.

Form CPFIS/G-ETF

This form allows you to apply for the inclusion of Gold Exchange Traded Fund under CPFIS-OA.
